Forms for the Summer 2008 Swim team are available below as downloadable pdf's. Please note that the Swim Team Release form is needed for EACH child swimming while the other 3 forms only ONE per family is needed. We will be accepting registrations beginning the first day of practice on May 27 - May 29. Please download the forms and bring with your check. We will have forms available on-site as well. We'll be at the pool from 4:30 -6:30.
